Recovery Connection Care Coordination

Recovery Connection is a care coordination service of the Monroe County Office of Mental Health. Care Coordinators are available through Catholic Family Center, Restart Outpatient and Rochester General Health Systems-Rochester Mental Health Center to clients with substance use disorders/co-occuring disorders in Monroe County. This program is designed to improve recovery outcomes for individuals who have received frequent detoxification or other intensive levels of service, but have not yet been successful in linking to any sustained addiction services. Retention in treatment is associated with better outcomes.

Benefits of participation in the program include: support and guidance with housing, transportation, obtaining benefits, legal issues, linking to primary healthcare, mental health, and vocational services. Care Coordinators provide these services within a collaborative relationship that is engaging, respectful and empowering.

Who is eligible for Recovery Connection?

Monroe County Office of Mental Health criteria for eligibility are:

  • An alcohol or drug dependence diagnosis of at least two years’ duration
  • Three or more admissions to detoxification, inpatient substance use or psychiatric treatment or other acute services such as emergency room care in the past 12 months, or four or more admissions in the past 24 months
  • Adults 18 years of age or older
  • Resident of Monroe County
  • Meets eligibility for Medicaid insurance

What are the expected outcomes?

Our case manager, together with participants and providers, will achieve the following desired outcomes:

  • Successful engagement into the recovery process
  • Discontinued/reduced use of alcohol and drugs
  • Successful reintegration into community living
  • Improved employment status
  • Reduced Medicaid costs
  • Reduced reliance on public assistance
  • Decreased days of homelessness
  • Decreased days in jail or prison

Rapid Engagement Demonstration (R.E.D.) Project

Rapid Engagement is a service of the Monroe County Department of Human Services/Office of Mental Health. The Engagement Facilitator is available through the Office of Mental Health to individuals in Monroe County who have a substance use/co-occurring disorder. The program is designed to make recovery possible for individuals who have had frequent unsuccessful DHS application attempts and/or use of Emergency Housing services. 

Benefits of participation in the program include: Motivational coaching, benefits advocacy, housing assistance, and support in linking to behavioral health, medical, transportation, legal and vocational services.

Who is eligible for R.E.D.?

Monroe County Office of Mental Health criteria for eligibility are:

  • An alcohol or drug dependence diagnosis of at least two years’ duration
  • Adults 18 years of age or older
  • Resident of Monroe County
  • Three or more or any combination of DHS applications or use of DHS Emergency Housing in the past 12 months
  • Eligible for Temporary Assistance

What are the benefits?

Our Engagement Facilitator, together with participants and providers, will achieve:

  • Engagement and successful retention of recovery serves
  • Discontinued/reduced use of alcohol and drugs
  • successful completion of DHS applications
  • Reduced Medicaid and Public Assistance costs
  • Decreased days homeless, in jail, prison, hospital, detoxification center or emergency housing
