Due to the Covid-19 pandemic all offices are currently closed and the Monroe County Auto License Bureau will be introducing a new method of requesting accomodations as it is in the process of replacing its appointment booking system.

Accommodations for
Individuals with Disabilities

If you require an accommodation to complete a transaction at a Monroe County Auto License Bureau-operated DMV office, including taking an audio-based instead of written permit test or require an American Sign Language interpreter, our staff members are happy to assist you.

Requesting an Accommodation

You can use our online appointment scheduling system to request an accommodation and set up a time to come in by visiting and selecting “Request for Accommodation” under “Service Type.”  Please make sure to provide contact information so that we can follow up with you if we need more information or need to work with you to come up with a solution.  Telephone access for accomodations can be made by calling the Clerk's Office at (585) 753-1600 and selecting option 3 for DMV, and then option 3 for accommodations.

Forms and Information

Every form used at Monroe County Auto License Bureau offices is available on the New York State DMV website at, where you can search by the title of the form or by keyword.  They are also available at our DMV offices.  There are over 500 forms in use by state DMV, but many of these are for very specific and unusual situations.  The overwhelming majority of DMV transactions rely on a handful of common forms:

  • MV-44 is the form used for both issuing and renewing learner’s permits, driver licenses, and non-driver ID cards.
  • MV-82 is the form used for registering or renewing the registration of a motor vehicle.
  • DTF-802 is the form used to pay sales tax on a newly-purchased vehicle
  • ID-44 describes what proofs of identity you need to be issued a driver's license, permit, or non-driver ID card.
  • MV-912 is a fill-in-the blank form you can use to create a Bill of Sale when you sell a vehicle. (Handwritten bills of sale are also still perfectly acceptable.)
