Application Forms

(jump down to Exam Study Guides)

Each exam requires an individual application form. For help completing an application refer to the Application Completion Guidelines.

Exams require a non-refundable application processing fee due at time of application. Credit cards are accepted online only. Checks or money orders are accepted in person. Cash is not accepted. In some cases the fee is waived due to financial hardship. See the exam announcements for details.

If you have applied for examinations offered on the same date as another civil service commission (ex. City of Rochester, New York State or another County), you must complete a crossfiler form.

If the exam you are taking requires a qualifying typing or stenographic performance test and you have already completed the test, please read and complete a Test Waiver Form. The exam announcement will inform you if a perofrmance test is required and the minimum words per minute required to qualify.
