Request for Qualifications - Home and Community Based (HCBS) Children's Waiver Intensive Care Coordination (ICC) Services click here

There has been a change in current providers of HCBS Waiver Children's Services, thus making eighteen (18) existing Children's Waiver slots now available for other community agencies to apply for. Monroe County Office of Mental Health (MCOMH) will recommend one applicant to New York State Office of Mental Health (NYS OMH), who will make the final selection of the provider for these slots.

Proposals Due from Respondents: Monday, May 21, 2018 (by 3pm)
Awards Communicated by MCOMH Friday, June 8, 2018

The Community School Behavioral Health Specialist position is designed to offer individualized, culturally relevant, and trauma-informed planning support for youth with a serious emotional disturbance (SED) or significant mental health challenge within a community school setting (more specifically, Enrico Fermi School No. 17). The primary focus of this service is to provide specific staff development, consultation and integrated planning between school, families and community providers to better address holistic needs.

Skill Building Services For Youth With Complex Mental Health Challenges click here
Selected Partners on 5/15/17: Hillside Children's Center, Villa of Hope & Compeer Rochester, Inc.
The primary focus of the Skill Building service is to work with children and their families to implement interventions outlined in a plan to compensate for or eliminate functional deficits and interpersonal and/or environmental barriers associated with a child/youth’s behavioral needs.

Family Support Navigators Team (FSNT) click here

Selected Partner on 2/27/17: Huther Doyle

The primary focus of the Family Support Navigators Team is to partner and engage with families and individuals impacted by substance use disorders, providing both education on the progression of addiction, how to access the service delivery system, what to expect when accessing treatment services, and fostering linkages to services.
