Monroe County is working with the Community Integration Initiative (CII) team to assist in the facilitation of this project. CII is staffed by TogetherNow.

TogetherNow was chosen as the project management team in this work for their expertise in facilitating a Human-Centered Design process, as well as their current work across a diverse network of committed providers and community members to put people at the center of care in an interconnected system of education, health, and human services.

Human-Centered Design is a unique approach to problem-solving which starts with the people you’re designing for and ends with new solutions that are tailor made to suit their needs.

The American Public Human Services Association (APHSA)

The American Public Human Services Association (APHSA) is a bipartisan, nonprofit organization representing state and local health and human service agencies. APHSA’s Organizational Effectiveness (OE) Team partners with health and human Services at all levels – systems, organizations, departments, and individuals – facilitating results towards the agency’s desired vision. The OE team has experience with over 140 projects with federal, state and local Health and Human Services (HHS). 

As a contracted partner for the CII, the Organizational Effectiveness Team supports the project’s change management activities using the Human Services Value Curve framework.

APHSA’s OE Team is at the forefront of translating the lens of the Human Services Value Curve into practice and application around the country. The Human Services Value Curve is a framework for public-sector transformation used for improving human services outcomes, value and legitimacy. Through the CII, a workgroup of DHS staff and leadership will be trained to apply the lens of the Value Curve in their work. Visit www.aphsa.org for more information.
